A confined space is large enough to enter and perform work, has limited means of entry or exit, and is not designed for continuous occupancy. In construction, common confined spaces include manholes, tanks, vaults, pits, tunnels, and large-diameter pipes. OSHA 1926 Subpart AA covers construction-specific requirements.
A permit is required when the confined space contains or has the potential to contain a hazardous atmosphere, engulfment hazard, converging walls, or any other recognized serious safety or health hazard. The template includes a hazard assessment to determine permit requirements.
At minimum, you must test for oxygen (19.5-23.5%), flammable gases (below 10% LEL), carbon monoxide (below 35 ppm), and hydrogen sulfide (below 10 ppm) before and during entry. The template provides continuous monitoring log fields for all four parameters.
