What a Hospital Daily Report Has to Capture That a Generic Template Never Will
Most hospital construction is renovation inside a building that never closes. The fourth-floor OR suite you are gutting shares a return-air plenum with an occupied med-surg unit, the corridor outside your barrier is a patient egress route, and the nurse manager two doors down has authority to stop your crew if the sticky mat is dirty. The daily report on this kind of job is not a production log with a safety paragraph bolted on. It is the written proof, page by page, that you built next to live patient care without putting a single patient at risk. When an ICRA breach is alleged, or a surveyor asks how a smoke barrier was protected while the sprinkler loop was drained, the daily is the document everyone reaches for.
That is why this template is structured around the infection-control and life-safety record first, and around trades second. Every day carries the ICRA class for each active work zone, the barrier walkdown, the negative-air and HEPA readings, the interim life safety status of any impaired system, and the facilities coordination that let you shut something down without a code being called on the floor above.
Containment and Air

The owner's infection preventionist and facilities director read it daily. The AHJ reads it at every inspection. The Joint Commission surveyor and the CMS validation team read it after the fact, and they read it against the FGI Guidelines, the ICRA matrix the owner signed, NFPA 99 for health care facility systems, NFPA 101 for life safety, and ADA for every patient-accessible space you touched. A daily report that only lists crews, weather, and quantities gives none of them what they need.
The ICRA and ILSM discipline
There is no tolerance for an infection-control breach on an occupied floor, so the discipline is unglamorous and daily: the same person walks the same barriers, reads the same manometers, signs the same line. The moment a fire alarm zone, a sprinkler branch, or a smoke damper is impaired, the report flips to ILSM mode and records the compensating measures in force (fire watch rounds, extra extinguishers, alternate egress posted) until the system is back and retested. Every shutdown, whether it is a med-gas zone valve or a domestic water riser, is logged with the facilities contact who approved it, the units notified, and the time service was restored. Write the report this way from the first day of mobilization and the survey is a formality. Write it like a generic template and you will be reconstructing it from memory, under scrutiny, for weeks.
